Limitless Physiotherapy and Pilates operates at 136 Macpherson St, Bronte, NSW under the ABN: 46608786349 and will be referred to as LIMITLESS. This form must be read, understood, correctly completed and accepted as indicated by properly signing, prior to participating in any LIMITLESS activity. If you are under 18 referred to as a Minor, you will need to have a parent or guardian who is over 18 sign on your behalf and provide contact details. Access to LIMITLESS activities is by payment and subject to LIMITLESS discretion. By participating in any LIMITLESS activity you accept the rules and regulations including participating in a safe manner.
Rules and Regulations:
The LIMITLESS activities, whilst care is taken, are not without risk which I/we accept, and in turn indemnify in full, on a solicitor and client basis, LIMITLESS, it’s Directors, employees, contractors, instructors, agents and nominees against all liabilities, damages and costs arising from LIMITLESS activities. This includes but is not limited to, liabilities arising from any cause such as negligence, contract, statute including the Civil Liability Act 2002, tortious or common law for any injury, death, loss, property damage or expense arising out of or sustained to me or I cause or my Minor causes, in the course of my or my Minor’s participation in the activities and programs offered by LIMITLESS, both on and off of the LIMITLESS premises or in relation to any activities, including to any party. Such activities include Physiotherapy, Clinical Studio Exercise sessions and Clinical Reformer classes. I agree that LIMITLESS is not responsible in the event of loss, damage, unauthorised use, theft or injury resulting from and to any personal property that my Minor or I bring onto or undertake on the premises. I also confirm that my or my Minor’s health is sufficient, and has no illness, injury or ailments which will prevent participation in LIMITLESS activities in a safe manner without risk to me or the Minor or others. I recognise that this Agreement and Release and Waiver is a legal instrument and that it is intended to be as broad and inclusive as permitted by Australian law. LIMITLESS’s liability in the aggregate is the refund of the fees paid for the LIMITLESS activity. I recognise that by signing this document I am waiving certain legal rights, including the right to sue and I sign it of my own free will and assume all aspects of the risks involved on behalf of my Minor or myself

Clinic Cancellation Policy
Please note we have a CANCELLATION POLICY and an ACTIVE WAITING LIST. If you are unable to attend your appointment, please give us at least 24 hours notice so that another client on the waiting list can be treated or fill your spot. You can cancel your appointment by phoning (02) 8970 9166 or emailing info@limitlessphysio.com
If you are cancelling outside our clinic hours please do so via email as we do not check the voicemail until we arrive in the morning.
We ask that you remember that a broken appointment hurts three…YOU, ANOTHER CLIENT who misses out and YOUR PHYSIOTHERAPIST who does not get paid for the missed appointment.
WE DO NOT LIKE CHARGING CANCELLATION FEES, so we please ask you to give us adequate notice and if we are able to fill your spot, you will not incur a charge.
Our cancellation policy is as follows:
If you cancel any session with MORE THAN 24HRS notice you will not be charged.
Cancellation with less than 24hours notice of Physiotherapy appointments, Private or Duo Pilates sessions will result in an $80 cancellation fee.
Cancellation with less than 24 hrs of Reformer Classes will be charged in full or result in losing a session from your pack.
Missed appointments will also be charged in full. A missed appointment means failing to attend your session or cancelling with less than 3 hrs notice making it impossible for us to fill your spot.
Please note ALL Reformer Class scheduling is self-managed through the Mindbody App. Please ensure you make any changes MORE THAN 24HRS before your appointment to avoid being charged. NO Reformer bookings or amendments will be made through the front desk.
